LIABILITY OF RELEASE FOR SETH BROUDY’S SCHOOL OF SURF

EXPRESS ASSUMPTION OF RISK

This is an important legal document. You must read, understand and sign this agrement to participate in Seth Broudy’s School of Surf activities. By signing this you waive certain legal rights, including your right to sue. I, the parent, guardian, or adult affirm and acknowledge that I understand the inherent risks and hazards associated with surfing and all the ocean and shore activities conducted by Seth Broudy’s School of Surf. I understand the hazards and risks can lead to severe injury or possible death. Despite the potential hazards and dangers associated with the aforementioned activities conducted by Seth Broudy’s School of Surf, I wish to freely accept and expressly assume all risks, dangers, and hazards which may arise from these activities and which could result in personal injury, death, and property damage to myself and my child or ward. In consideration of being of being allowed to participate in Seth Broudy’s School of Surf as well as the use of any of the facilities and equipment of the below listed Releases, I hereby agree as follows:

1. To waive and release any and all claims I may have in the future against any of the following named persons or entities (hereinafter reffered to as Releases): All instructors, including Seth Broudy’s School of

Surf, it’s staff, volunteers, and board its officers, clients, employees, and agents.

2. To release and hold harmless the Releases, their officers, directors, employees, representatives, agents, and volunteers from all liability and responsibility whatsoever, for any claim or cause

of action (lawsuit) that I, my estate, heirs, executors, or assigns may have for personal injury, property

damage or wrongful death arising from surfing and all the ocean and shore activities conducted by Seth

Broudy’s School of Surf whether caused by the active or passive negligence of the Release or otherwise.

3. All photographs and video taken of participants are sole property of Seth Broudy’s School of Surf and are subject to future use in any advertisement or media formats.

4. Acknowledge, I am not relying on an oral or written representations or statements made by the Release

other than what is set forth in this agreement.

5. This Agreement shall be governed by and interpreted in accordance with the laws of the Commonwealth of Virginia.

6. In the event of weather related cancellations, we will offer alternative camp dates within the season schedule.

By reregistering, you agree this agreement.  I confirm I have read it, and I agree to be bound by it
